C-Tech Credit Application

There are 3 parts to this credit application.  Please be sure to fill out all forms in their entirety.

1. Part One - Application
2. Part Two - Credit References
3. Part Three - Resale Certificate

Once the forms have been filled out, and any appropriate documentation has been attached, please do the following:

1. Sign the original, printed documentation
   
   
2. Mail the original to:

3. If you would like to expedite your credit application, you may fax your application to: 952-249-6859 - ATTN: Accounting - Credit Approval
   
   Please be sure to follow up the fax or email copies by sending the original signed documentation in the mail.

Credit applications take approximately 10 working days to approve, barring any delays in the response of your credit references.
